An associate of mine has an HSA account through ACS/Mellon via an employer.  It started out just fine, but they kept lowering the interest over the course of 3 years to the point where it's now 0.25%!  That doesn't cover the fees they charge.

After being told by ACS/Mellon that he couldn't have another HSA that the 'LAW' only allows you to have one... he finally was able to wrangle a transfer out form from them.  Took talking to a manager to get anywhere.

 So, if you want to do a trustee to trustee transfer OUT of ACS/Mellon into an HSA that actually returns reasonable interest, you can download the form.

 It's really rather pathetic when the customer service people lie to try and keep your money working for them and not you.  It'll cost you $25 (according to this form, however check your schedule of fees).

Follow Up: The transfer completed without a problem, cost $25 in this case and now the it'll make that money back in less than 6 months at 1.25% with $0 fees from hsabank.com.

 I hate to sound like a spamming advertiser, but I felt it important that people know they are not locked into the crappy HSA account that may be offered by an employer.  Shop around.
